This is a great opportunity to join a Nurse led team. You will operationally manage a team of nurses and health care assistants and be accountable for a caseload of patients. You will ensure effective, efficient, safe and high quality nursing assessment, treatment and delivery.
As the named District Nurse for a GP practice you will work closely with GP’s, Practice Nurses, social care, local hospitals, specialist nurses, therapists, adult social care, hospices and palliative care teams, mental health services, the voluntary sector and others to help individuals manage their own conditions, maintain their independence and remain at home wherever possible.
Current and relevant experience as well as further qualifications and evidence of continuing professional development is essential.
