Nurse Case Manager – Part Time (Full time also potentially available)

The Role

Our Client specializes in supporting children and adults with complex care needs in the community. We are seeking a dedicated Nurse Case Manager to join their established team at the Kent branch, serving the surrounding areas.

Key Requirements

  • Current and valid NMC Pin
  • Valid UK driving licence with access to a vehicle
  • Previous experience in managing and training staff members

Important Details

  • Shift Pattern: 3 days per week, 9am-5pm
  • Salary: £25,708 per annum with a potential quarterly KPI bonus (OTE £27,764)

Responsibilities

  • Manage and provide clinical leadership for a caseload of Nurses and Care Support Workers
  • Collaborate with the office team to effectively plan workloads and duties
  • Support daily operations of the Kent branch, including administrative tasks
  • Facilitate Registered Nurse and Care Worker development through clinical training, supervision, and competency sign-offs
  • Serve as a liaison between commissioners, clients, and the company, coordinating with other professionals as needed
  • Provide clinical leadership and prioritize individual client needs through regular assessments, planning, implementation, and evaluation of care plans
  • Conduct client visits to offer clinical support and guidance to staff members
  • Attend review meetings and adult/child protection meetings with Care Managers, Social Services Managers, and Primary Care providers

Ideal Candidate

  • Experience as a Nurse Case Manager or in a similar role within the care industry
  • Computer literate with strong literacy and numeracy skills
  • Understanding of CQC regulations and care provision legislation
  • Full UK driving licence
  • NMC Pin (qualified for over a year)
