This Role offers a block booking/shifts for up to November 2024, 5 Days per week; Hours 9am to 5pm

Responsibilities:

• Provide skilled nursing care to patients in their homes, delivering personalized care plans tailored to individual needs.
• Conduct assessments, monitor patient progress, and adjust care plans as necessary to ensure optimal outcomes.
• Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to coordinate comprehensive care for patients.
• Educate patients and their families on managing health conditions and promoting wellness within the community.
• Maintain accurate and up-to-date patient records in accordance with professional standards and regulatory requirements.

Qualifications and Requirements:

• Registered with the Nursing and Midwifery Council (NMC) as an RGN.
• Previous experience in community nursing or a related field is desirable but not essential.
• Excellent communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
• Strong clinical skills and a commitment to delivering high-quality, patient-centered care.
• Valid driver's license and access to a vehicle for community visits (if applicable).
