Your role -

• Full time frailty ANP position
• Support with Face-to-face consultations
• Brilliant development opportunities
• Support in areas such as frailty, minor illness, injury, minor ailments and acute on the day appointments
• See long term conditions and exacerbation of them
• Working collaboratively with the multi-disciplinary general practice team
• Undergo patient triage and telephone consultations
