Are you a motivated, experienced Care Coordinator looking for an exciting opportunity to make a real difference? Our client, a leading UK provider specialising in Nursing and Care services for children and adults living with complex physical disabilities and life-limiting conditions, is on the lookout for a dedicated individual to join their team.

Our client delivers nurse-led services in the clients' own homes and works in partnerships with various healthcare organisations to improve patient outcomes and experiences. Their focus is on leading with quality and putting their clients at the heart of everything they do.

As a Care Coordinator, your responsibilities will include:
  • Coordinating and overseeing the mobilisation of new complex care packages
  • Managing the process of new package set-up including care plans, risk assessments, and compliance with regulatory requirements
  • Developing relationships across the business to ensure the bespoke service runs seamlessly
  • Liaising with key stakeholders including clients, families, healthcare professionals, and internal teams to ensure a smooth and timely delivery of care

The ideal Care Coordinator candidate will have:
  • Previous experience in care package coordination
  • Proven leadership skills

  • Strong organisational skills
  • A flexible and creative problem-solving ability
  • Numeracy skills and the ability to analyse complex issues, absorb information and understand data
