Are you a dedicated and compassionate Clinical Pharmacist seeking a fulfilling role within a forward-thinking, innovative, and progressive Practice?

We have partnered with a well-established training practice in rural East Sussex, who work in partnership with a social enterprise organisation who are a leading provider of urgent care in the South-East. Based at a unique, purpose built, eco-surgery, they have a list size of approx. 5,000 patients and are committed to delivering high quality, integrated primary care to support their patients to live their lives to the full.

Position Details:

  • Job Title: Clinical Pharmacist
  • Job Type: Full time/part-time hours available from Monday – Friday
  • Location: Wealden District of East Sussex

  • Team: Supported by GP Partners, salaried GPs, Care Co-ordinator, Social Prescriber, Mental Health Practitioners, Pharmacists and ANPs plus clinical administration team.

About You:

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Experience as a pharmacist, demonstrated within a practice portfolio.
  • An awareness of common acute and long-term conditions.
  • May hold or be working towards an independent prescribing qualification.
  • Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate.
  • Excellent interpersonal and communication skills, and be organised, patient and empathetic.

Main Duties:

  • Conduct long-term condition clinics and clinical medication reviews.
  • Make recommendations for medicine optimisation and safety.
  • Support staff with prescription and medication queries as well as manage repeat prescriptions and monitoring tests.
  • Lead medicines optimisation and quality improvement.
  • Provide training on therapeutics and medicines optimisation and critically evaluate literature and ensure compliance with CQC standards.
