This is a genuine opportunity to help others within the Bury community to support Community Hospital Care. As a Emergency Response Coordinator, Rapid Response your vocation will be to work with vulnerable adults and help them build confidence, support health and wellbeing so they reach potential and can live full and independent lives. This is a rapid response role that will have you out in the community supporting adults with physical disabilities due to illness or injury, this is a complex role and a Level 2 or higher is required.

A van will be provided by the service for the pick-up service, along with adequate equipment to assist with lifting people who have fallen. You must also have a drivers licence and access to your own vehicle, as we may require you to perform care visits during downtime. Free parking is available on site.

For this position you will need to have progressed though Level 2 in Social Care and be ready to take on additional responsibilities, ideally you will have worked in a similar role before and are looking for that progression. You will be working with an experienced team of multi-disciplinary health and care professionals including Occupational Therapists, Physiotherapist that are all there to support reablement of vulnerable adults and help them to achieve their personal goal plan. You will be required to evaluate and modify programmes and recognise improvements and maintain clear and accurate records using company mobile phones, lap tops and computers.

You will be required to attend multi-disciplinary team meetings and will include liaising with and acting on advice provided by physiotherapists and occupational therapists.

You will need to be a team player who supports, assists and promotes the reablement of customers with identified needs, therapeutically and pro-actively enabling them to reach their own potential, maintain Independence and continue living within their own homes after a concentrated support period.

You will be required to support emergency planning activities by assisting managers to set up and run rest centres when required in emergency situations.

Choices for Living Well - Intermediate Rapid Response Service

We are a community run, interim service that supports people, many with complex needs due to illness or injury, to regain their skills and confidence so they can return home after a period of therapy and support. We offer an interim safe space in a lovely, newly renovated residential house and extend our services out to those who have returned to their own homes. This is short term service, and you will positively contribute to the assessment of customers' needs to create an accurate therapeutic reablement programme. You will assist and support customers, including those with complex needs and behaviours with active daily living skills such as personal care, and skills that will focus on them remaining independent in their own homes.

You will be required to keep clear and accurate records as per the Social Care Act 2008 this will be achieved by using company mobile phones, lap tops and electronic monitoring equipment.

Requirements

FULL TIME ROTA as follows

· This will be a full-time position on a rota basis, between the hours of 8am and 10pm, Monday to Friday. 37.5 hours per week.

· Council benefits package.

· Pay Grade 7 hourly between £12.59-£13.47.

· You will have access to a van and equipment, provided by the service, to assist with lifting people when they have fallen.

· Free on-site parking.
